COURSE DETAILS
Enrollment Requirements:
•Proof of minimum age 18 or older (State of Colorado requirement).
• Proficiency in reading, writing, and speaking English (State of Colorado requirement).
• Basic math skills (State of Colorado requirement).
• Photo identification required at the one-on-one Zoom meeting with the Nurse Instructor (State of Colorado requirement).
• Pre-payment for the course (all fees are non-refundable).

QMAP Academy's Fee:
• The fee is $125 per student, to be paid prior to class. Payment options are provided at registration.

Course Details
1. This course is a self-paced online class and includes training videos, practice quizzes, training guide, downloadable material and final virtual written exam.
2. The final practicum exam with the Nurse Instructor is required to be in person.
3. Upon completion, students receive a Certificate of Completion and placement on the State of Colorado QMAP verification site.

Self-Paced: Students manage the course independently, including the videos, practice quizzes, and final exam. Students must affirm they are ready to move to the next section. This course can be taken at the student’s own pace, allowing sections to be repeated, though skipping ahead is not recommended.
Final Exam: Timed, with a passing grade of 80% or above. Students have two attempts to pass. Questions are randomized, meaning questions are in different order for each student. The final exam should not be opened unless the student is ready to complete it, as opening it counts as an attempt.
Final Practicum: Scheduled in person, typically for 30 minutes between 9 AM and 5 PM on the identified date. The student will be asked to demonstrate QMAP functions and explain QMAP duties. Legal ID matching registration details must be shown.

At the successful completion of the course, students will be able to:
• Demonstrate proper reading, understanding, and transcribing of physician’s orders.
• Safely administer medications via multiple routes using written physician orders according to the "7 rights of administration."
• Document medication administration in accordance with state board of health regulations.
• Safely administer medications from Medication Reminder Boxes (MRB).

Student Responsibilities:
• Complete the course within the timelines identified at registration. Failure to do so may result in failing the course.

• QMAP Academy is an Approved Training Entity (ATE), and our curriculum is approved by the Colorado Department of Public Health and Environment for these settings only:
• Assisted living residences
• Adult foster care facilities
• Alternative care facilities
• Residential child care facilities
• Secure residential treatment centers
• State certified adult day programs
